Parkinson's Disease is estimated to affect around 4 million people worldwide and is characterised at the molecular level by the presence of Lewy Bodies in the dopaminergic neurons of the brain. These Lewy bodies have been shown to contain fibrillar deposits rich in a-synuclein, a protein whose function has yet to be fully characterised.

In Parkinson's disease the protein forms insoluble fibrillar deposits which are clearly toxic to the cell, affecting neuromuscular signalling as a result of disruption of the factors controlling synaptic dopamine homeostasis. Certain pesticides such as rotenone and paraquat have been shown to promote fibrillar formation both in vivo and in vitro.

It is clearly important to explore the role of a-synuclein in the dopaminergic neurons in order to understand the aetiology of Parkinson's disease. By characterising the factors involved in the conversion from soluble to insoluble forms of a-synuclein we will be able to gain an understanding of this process in other proteins prone to fibrillar formation. In doing so it should be possible, in the longer term, to develop meaningful strategies which lead to therapeutic solutions to such problems.

After a time on medication, patients may notice that each dose wears off before the next dose can be taken wearing-off effect or erratic fluctuations in dose effect on-off effect. Surgery is required to place a small hole stoma in the stomach through which a tube is connected to a portable pump worn on your belt.

It is designed to deliver the medicine continuously, a little at a time, to improve absorption and reduce off-times. Duopa is similar to insulin pumps used by diabetics. Surgery When medications fail to control symptoms because of severe "on-off" fluctuations, lack of effectiveness, or intolerable side effects, surgery should be considered.

Other surgical procedures include pallidotomy and thalamotomy. Patients with severe depression, advanced dementia, or an unstable medical condition may not be candidates for surgery. Also, patients who have symptoms similar to PD but who have been diagnosed with a different disorder, such as multiple system atrophy, progressive supranuclear palsy, or cortical basal degeneration should not consider surgery.

Clinical trials are research studies in which new treatments—drugs, diagnostics, procedures, and other therapies—are tested in people to see if they are safe and effective. Research is always being conducted to improve the standard of medical care.

That cell is stimulated to pass the message on. After the message is passed on, the receptors release the dopamine molecules back into the synapse, where the excess dopamine is "taken up" or recycled within the releasing neuron. The stimulator settings are programmable and can be adjusted as your symptoms change over time.

DBS improves the symptoms of slowness, tremor, and rigidity. Most people are able to reduce their medications and lessen their side effects, including dyskinesias. Furthermore, DBS does not damage the brain tissue. Thus, if better treatments develop in the future, the DBS procedure can be reversed.

Other surgical procedures include pallidotomy and thalamotomy, which target specific brain areas. But instead of stimulation, a high-frequency energy current is used to permanently destroy the cells. These procedures are not reversible and are only used in special cases when a DBS is not feasible.
